The Power of the Sugar Pill

When a medicine is prescribed or administered to a patient, it can have several effects. Some of them depend directly on the medicine's pharmacological action. There exists, however, another effect, that is not linked to the medicine's pharmacology, and that can also appear when a pharmacologically inactive substance is administered. We call it "placebo effect". It is one of the most common phenomena observed in medicine, but also a very mysterious one.The placebo effect is powerful. In a study carried out at the University of Harvard, its effectiveness was tested in a wide range of disturbances, including pain, arterial hypertension and asthma. The result was impressive: 30 to 40 percent of the patients obtained relief with the use of placebo. Furthermore, the placebo effect is not limited to medicines but it can appear with any kind of medical procedure.In a trial to test the value of a surgical procedure (ligature of an artery in the thorax) to treat angina pectoris (pain in the chest caused by chronic heart ischemia), the placebo procedure consisted in anaesthetizing the patient and only cutting his skin. The thus fictitiously treated patients showed an 80 percent improvement while those actually operated upon only 40 percent. In other words: placebo acted better than surgery.
What is the placebo effect? How can it be explained?
The knowledge about the placebo effect was very much enlarged with the medical need to perform controlled clinical trials,a scientific methodology largely used to determine the therapeutic effectiveness of new medicines. In these trials a placebo is obligatorily administered to a control group of patients, and later the results of this group are compared with those obtained in the group that receives the active medicine, whose action one intends to demonstrate. The bigger the difference in the results between the second and the first groups, the bigger the pharmacological effectiveness of the studied substance.Very soon these studies made doctors notice that placebos had much more effects upon the studied diseases than anticipated. In some cases the placebo
side-effects outreached those of the active medicine. Consequently, there was a great increase in scientific researches aiming to clarify what is this effect, why does it happen, what is its physiological bases, etc . Since the placebo effect can be a real one, calling forth beneficial changes in the patient, it can be very useful in clinical practice.
The Healing Power of Placebos by Tamar Nordenberg
One patient stands out in the memory of Stephen Straus, M.D., for her remarkable recovery, more than 10 years ago, from chronic fatigue syndrome. The woman, then in her 30s, was "very significantly impaired," says Straus, chief of the Laboratory of Clinical Investigation at the National Institute of Allergy and Infectious Diseases. "She had no energy, couldn't work, and spent most of her time at home." But her strength was restored during a study to test the effectiveness of an experimental chronic fatigue drug. "She and her parents were so thrilled with her recovery that they were blessing me and my colleagues," recalls Straus, the principal investigator on that study.
Turns out, the woman's quick turnaround from chronic fatigue occurred after taking placebo pills, not the experimental drug. Straus says, "She was amazed by the revelation that she'd gotten better on placebo."
Like many drug studies, the chronic fatigue medication trial was a "placebo-controlled" study, meaning that a portion of the patients took the experimental drug, while others took look-alike pills with no active ingredient, with neither researchers nor patients knowing which patients were getting which.

When 26-year-old Jennifer Kennedy (not her real name) from Rockville, Md., has a panic attack, she simply distracts herself with a book or crossword puzzle. But at their height, several years ago, the attacks scared her so much that she finally went to the emergency room. "I thought I was going to die from a heart attack," says Kennedy. She didn't believe her doctor when he told her to "just relax" and she would feel okay, and he ended up prescribing pills. He didn't tell her until two weeks later, after her panic attacks had subsided, that the pills were simply placebos with no active medical ingredient.
Kennedy admits to feeling deceived when she first found out the pills her doctor prescribed were fake, but says she now appreciates the doctor's decision to prescribe the placebo. "At first, I felt stupid. But that day at the hospital, the doctor must have realized I wasn't going to accept 'you're fine, it's all in your mind.' The placebo helped me realize that I'm not unhealthy and I'm going to be okay. Now, I think it's really neat that something that didn't really affect me had such a big effect on my life."

HYPNOSTORIES
Our thoughts are extremely powerful and can have devastating results when used incorrectly. You can make yourself sick, poor and unhappy by the nature of your habitual thinking; you can even kill yourself by your thoughts as shown in the four following case histories. (The following stories can be verified through books at your local library.)
In the latter part of the 19th century, a physiologist named Boorheave requested the courts to assign a condemned criminal to him for experimental purposes. In the presence of interested observers, he placed his victim on a stone slab, blindfolded him, and pretended to draw blood from his veins while buckets on either side of him received drops of water to simulate blood pouring from his body. A few minutes later, the blindfold was removed, and when the observers bent over to examine the unusually pale body of the prisoner, they discovered he was dead.
A railway inspector entered a refrigerated rail truck while on his rounds checking the roadworthiness of railcars in the yard. While he was occupied, the door of the truck closed and despite all his attempts to open the door, it wouldn't budge. He considered his position logically. It was after lunch on the last working day prior to a four day long weekend. Because he worked by himself, none of his workmates knew where he was and he would not be missed for at least four days. Being a refrigerated car, he was aware of the danger, and knew it was impossible to survive more than six hours in this condition. So that his death would not be in vain, he wrote a detailed note telling how it felt to freeze to death with his last words being "It is very cold, I feel the end is near."
When his workmates opened the truck and found him four days later, they found he had overlooked one vital factor. The thermostat that controlled the temperature was damaged and would not cool down below 59 degrees Fahrenheit.
In a famous English legal case, a tutor perished in an odd manner. The boys at the school where he taught disliked him intensely and decided to "rag" him. They erected a makeshift guillotine in the schoolyard, and when it was completed, they captured the frightened instructor, covered his eyes with a cloth, and told him that soon the blade would release and chop off his head. One boy made a sound like a descending blade; another slapped him over the back of the neck with a wet rag. The man died instantly.
A man complained to his physician about insomnia and requested Phenobarbital. The doctor, knowing the mans case history, was more than a little concerned with the request and instead, prescribed vitamin C. (Vitamin C is water soluble and any excess amount passes straight through the system without causing any harm.) The next morning, the man's wife rang. She was very concerned that her husband was in a very deep state of sleep and could not be awakened.When the doctor attended, he found the man's breathing was shallow and the man was close to death.
